Driver Criteria

Please ensure you meet the following criteria before completing and submitting the following application form.

• Must be at least 25 years old

• Hold a valid AZ or DZ drivers license

• Provide a current copy of your Drivers Abstract

• Provide a current copy of your CVOR Abstract

• You must have no more than two violations or three points on the abstract

• Provide names and phone numbers of previous
  employers for reference
